We accept and are contracted with many insurance carriers.  If you have a question concerning your insurance carrier, please contact our office. We will file your insurance claims directly from this office.  Please bring all current insurance cards so that your claim can be filed with the correct insurance carrier.

Some insurance carriers require a referral authorization number for your visits as well as precertification for other scheduled test, xrays, and surgeries that you may need.  Please obtain the authorization and referral prior to your scheduled visit.

Unauthorized visits and procedures will most likely be denied payment by your insurance company and you may be responsible for the charges.

We require copays to be paid at the time of the office visit. If surgery is recommended and your insurance company has a deductible, we will collect the deductible before surgery.

At this time, we do NOT accept the following insurance plans:

1.    BlueCross BlueShield HMO (BlueChoice HMO)

2.    BlueCross BlueShield POS  (Blue Choice POS)

3.    Wellcare
