Please bring the complete office forms to our office with you on the day of your child's appointment. Faxed forms are difficult to read and often require filling out new forms in the office.

Existing Patients: You can save time by reviewing Pre-Operative and Post-Operative instructions. Please remember to inform us if any registration information has changed so we may make the appropriate updates. Each calendar year, we require updated complete office forms which can be downloaded and completed prior to your visit. A followup history form is used for all other follow up visits so we may have updates on your child’s medical history.
